Increased accessibility to health care is an initiative that can be achieved in various ways. First, utilizing mobile clinics to get to the patient and encouraging other health organizations to embrace it would be the first initiative that will help to increase accessibility to health care (Yu et al., 2017). Mobile clinics would be responsible for providing primary health care services and offering preventive health care services to patients that are unable to access health care setting. Secondly, increasing pop up clinics and campaigning for health care settings to organize pop up clinics would be another great initiative. Pop up clinics provide health service that range from preventive screening and treatments that are specific for a certain illness to several patients at little or no cost at times. Immediate health care needs of patients would thus be met especially for patients that cannot access health care.Putting up a health care task force within communities will make it make it easy for patients to access health care. Working closely with other stakeholders, will encourage health personnel to come together and provide health care services to communities as a team. Finding people and other vital organizations that will improve transportation services to the communities, hence enhancing the provision of health services (Ursulica, 2016). Additionally, teaming up with Independent Transportation Network will be crucial in providing health care services to people that are in places that are not able to get health care. Health care can be provided to residents in states that International Transportation Network operates. This transportation network will assist in taking health care services door to door to patients that cannot move.PEER 2The biggest issue currently in achieving Triple Aim and Universal Healthcare in United States is the access of primary care.  Poor access is attributed to many factors among them lack of insurance, accessibility of available services and most importantly lack of adequate primary care providers amidst the rising demand of care. APNs are playing a crucial role in improving access to primary care. APNs can collaborate with other primary care providers to form efficient interprofessional health teams that would reduce costs and time in accessing care to improve care access.  For example in future, as an APN, I can see myself liaising with physicians and other healthcare professionals to coordinate patient care, as well as, as help offer primary care from diagnosis to prescribing, which are among APN scope of practice. This is critical in reducing the time taken by patients to access care and reducing hospital readmissions to allow nurses and other healthcare professionals attend to more patients; these have a net effect on improving healthcare access both in terms of time and number of patients attended (Neto, Rewa, Leonello, & Oliveira,  2018).It is important to note that there are vulnerable populations that are most affected when it comes to healthcare access. Targeting these populations can have the overall effect in improving healthcare access and inequality in access to care. In future, as more state laws are adjusted to allow APNs full practice authority, I can see myself targeting the vulnerable populations in many ways in my position as an APN. Among them include designing health promotion programs targeting vulnerable, such as African Americans with diabetes and Hispanics with hypertension to minimize the predominant predisposition of these populations to the conditions noted. If I am allow to practice under full authority, as an APN, I hope to set a mobile clinic to be visiting people with chronic illnesses at their home places (home care) (Bryant-Lukosius et al., 2017). Particularly, people with chronic illnesses and disabilities, which are costly to manage and debilitating, are often neglected in care access due to logistical/transport challenges and drained finances. The target would be to offer them low cost care, by coming up with interventions to reduce care costs, complications and restore their functionality if possible. For example, there is room to offer primary care services, such as diagnosis (both secondary and primary) and interventions taking a holistic approach, such as use of low cost drugs, lifestyle changes, referral to support groups and engagement of support from family, friends and community members to help support patient in the care process (Maier, Budde & Buchan, 2018).
